Office Policies


All current patients have an emergency contact number. Sometimes patients need phone consultation, letters, or form completion as part of treatment. These services carry a charge if they require significant time.

Patients are asked to give 24 hours notice if they are unable to keep an appointment. Broken appointments will be billed except in unusual circumstances. Payment or copayment is expected at the time of service. Credit cards, checks and cash are accepted. I accept all Anthem plans except Healthkeepers.
